PHSP Enrolment Process
Getting started is fast and straightforward. Enrol online using our web form, or download and print physical mail-in forms.
Employer & Employee Signup Step 1
The employer initiates the signup process. You can enrol via our secure online form or download a paper application packet. Please prepare the following details before starting:
- Full legal corporate/business name and phone number
- Head office address and email address
- Corporate fiscal year-end date
- Void business cheque (required to automate claims via EFT)
- Full legal name, email, and mailing address
- Social Insurance Number (SIN)
Note: After completing the online registration, the employer faxes or mails a printed, signed copy of the PHSP Administrative Services Agreement and Approved Employees list to Promedent.
Enrolment Fee Processing Step 2
Promedent processes a one-time setup and registration fee of **$150 + GST/HST** (as applicable by province).
This payment is initiated securely via Electronic Funds Transfer (EFT) from the business bank account provided in Step 1. The setup transaction may take up to 30 days to process and establish connection with your financial institution.

Membership Activation Step 3
Once setup is complete, Promedent officially launches the plan and notifies each registered employee by email with their details:
- Plan welcome notification and enrolment approval
- Employer-defined annual healthcare spending limit
- Personal Member ID and system-generated temporary password
Employees can then log in to the secure client area to set a custom password and enter their personal direct deposit details for automated claims reimbursement.
